For years, readers have speculated about the mysterious woman behind The Housemaid — one of the most successful thriller series in recent memory. Now, the author has finally answered the question that has captivated her fanbase: who is Freida McFadden?

In a revealing interview published April 8, 2026, the bestselling author confirmed that her real name is Sara Cohen — and the life she leads outside of writing might be just as surprising as anything in her books.

A Double Life Between Books and Medicine

Sara Cohen, 45, is not only a prolific thriller writer — she is also a practicing physician who treats patients with brain disorders. For more than two decades, she successfully maintained two entirely separate careers, carefully guarding the line between her public persona as Freida McFadden and her day-to-day identity as a doctor.

"My whole goal was to keep it a secret until I was ready to step back from my doctor job, so it wouldn't compromise my ability to do my job," Cohen told USA Today.

What makes the secret all the more remarkable is that it held — at least for a while. Colleagues at her hospital eventually discovered who she was, but according to Cohen, they were "really nice about it" and kept her secret, much like other celebrities who keep surprising sides of their lives private for years. Many had been working side by side with a New York Times bestselling author without knowing it.

Why She Chose a Pseudonym

The decision to write under a pen name was a practical one from the start. Cohen feared that a public profile as a novelist could create friction with her medical career. She worried about how patients and colleagues might react — and she simply wasn't ready to navigate that conversation.

Writing was also, at its core, just something she loved. "Some people go into writing hoping to quit their day job, but I didn't do it for that reason. I was just having fun," she told Jenna Bush Hager on the Open Book podcast in December. "I kept clinging to being a doctor because I worked very hard to get there and I find it really rewarding. I love seeing patients and helping people."

The decision to wear a disguise in public appearances — including a wig and glasses — added another layer to the persona. Cohen does actually wear glasses, she clarified, but the hair was a prop. She acknowledged that her real life is "so much more boring than anything that happens in my books."

An Avalanche of Online Conspiracy Theories

The secrecy surrounding McFadden's identity inevitably fueled speculation online. Her lack of verifiable personal information led some corners of the internet to question whether she was even a real person. Cohen addressed the rumors with good humor.

"Some of them are just so out there. I think every author gets 'AI is writing her books,' even though most of them were written before AI," she said. "One that's very funny is people saying that I'm three men, and I think that's hilarious."

She also described a post in which someone claimed her author photo had been digitally altered. Fans of murder mystery thrillers were among the most intrigued by the real-life mystery surrounding her identity. The speculation reached such heights that she became a kind of enigma in book communities — which, for a thriller writer, is fitting.

The Emotional Cost of Hiding

While the secrecy served its practical purpose, it came with real personal costs. Cohen was unable to participate in author events or book tours, missing out on some of the most meaningful parts of connecting with readers.

"It's very hard," she said. "I see all these authors doing these huge book tours, which I never do, and I feel so bad that I never do it."

There were also moments of genuine overwhelm. "There was a moment when I was writing the third Housemaid book and moving simultaneously, and I was just losing my mind," she said. "I was almost having panic attacks." Despite those struggles, she found a way to sustain both careers — a feat that few could manage.

Ready to Be Known

Cohen says she has reached a turning point. She stopped working full-time as a physician in 2023, and as her career as a novelist has grown to extraordinary heights, the need for secrecy has faded.

"I'm at a point in my career when I'm tired of this being a secret. I'm tired of people debating if I'm a real person or if I'm three men," she said. "I am a real person and I have a real identity and I don't have anything to hide."

She plans to continue publishing under the name Freida McFadden. "Even though I haven't told my real name until now, I feel like I have shared the real me all along and everything I've told them has been the truth," she said. "I've always been genuine with my readers."

What's Next: The Housemaid on Screen

The timing of this reveal coincides with an exciting period for the franchise. When looking ahead at every major movie coming in 2026, The Housemaid franchise stands out as one of the most anticipated. The first film, starring Sydney Sweeney and Amanda Seyfried, was a major success — and a sequel, The Housemaid's Secret, is already in development with Sweeney set to return alongside Kirsten Dunst.

For those who love streaming the biggest films at home, the adaptation of McFadden's work is essential viewing. And for fans who have long wondered about the woman behind the page, the answer has finally arrived: she is Sara Cohen — a doctor, a writer, and someone who has always been exactly who she said she was.
