Mom Notices Newborn Smells Like Maple Syrup, Discovers Rare Disorder

Megan Popps noticed something unusual about her newborn son Leo shortly after bringing him home in February 2026. He smelled sweet. Not like baby powder or lotion — sweet like pancakes, like maple syrup. She and her partner Devaughn Debelak mentioned it to each other a few times. Leo seemed fine otherwise, so they didn't panic. But the smell didn't go away.

Then the hospital called. The routine heel-stick newborn screening test done before discharge had come back inconclusive. Something was off with Leo's amino acid profile. They needed to redo it.

That phone call sent Megan to Google. She started researching what an abnormal amino acid result in a newborn could mean. What she found stopped her cold.

Maple Syrup Urine Disease. MSUD. A rare genetic condition where a baby's body cannot break down three specific amino acids — leucine, isoleucine, and valine. When those amino acids accumulate they become toxic. And they produce a distinctive sweet smell — in urine, sweat, and earwax — that smells exactly like maple syrup.

The smell she'd been noticing for days wasn't random. It was a symptom.

Before heading to the ER, Megan packed some of Leo's clothes and a soiled onesie specifically to show the doctors. She wanted them to smell what she had been smelling. "There was no urine smell at all," she noted — just that sweet, pancake-like scent coming from her baby's body.

Doctors found his amino acid levels were critical. Leo was diagnosed with MSUD and hospitalized for three weeks.

"When I looked up the disorder and saw how rare it was, it was hard for us to grasp our brains around the fact that was what was possibly happening," Megan said. Leo is well now — but he will need to closely monitor his diet for the rest of his life.

What MSUD Actually Is

MSUD affects roughly 1 in 185,000 births worldwide. Most parents have never heard of it. Many pediatricians have never seen a case. That rarity is part of what makes it so dangerous — early symptoms are easy to explain away.

The disease is genetic, caused by a mutation that disables an enzyme responsible for breaking down three branched-chain amino acids. Without that enzyme, the amino acids accumulate. The buildup is toxic to the nervous system.

Classic MSUD begins showing signs within the first days of life. Early symptoms are subtle — poor feeding, a weak or high-pitched cry, lethargy, and that distinctive sweet smell. But without treatment, things escalate fast. The toxic accumulation can trigger a metabolic crisis where seizures begin, abnormal muscle movements develop, and coma follows. Without urgent intervention MSUD can be fatal as early as the first two weeks of life. Children who survive untreated metabolic crises often face permanent brain damage.

Treatment works when caught early. The primary approach is a carefully controlled low-protein diet that restricts the three problematic amino acids while providing all other needed nutrients through specialized medical formula. Some severe cases require liver transplantation, which replaces the defective enzyme pathway. With proper management, children with MSUD can live healthy lives. Everything depends on catching it in time.

Why Newborn Screening Changes Everything

Before universal newborn screening, MSUD diagnoses typically came after a baby was already in crisis — seizing, in a coma, or worse. Today all 50 states screen newborns for MSUD as part of the standard heel-stick test done before hospital discharge. That one test, done in the first days of life, is what gives families the window to intervene before a metabolic crisis hits.

It's what saved Leo. The inconclusive result triggered the callback. The callback sent Megan to research. The research made the smell make sense. And instead of waiting to see what happened, she packed a onesie as evidence and walked into an ER.

Paying attention to something as specific as your newborn smelling unusually sweet — and not dismissing it — gave her son his life.
